What is a USPS Form 1583?

The United States Postal Service or USPS Form 1583 is a critical form for any business that wants to use a third party, such as Starthub, to manage their mail and use mail forwarding and mail scanning services.  A third party such as this is referred to as a Commercial Mail Receiving Agent (CMRA).  The Form 1583 is the required form for customers to authorize a CMRA to receive mail on the customer’s behalf.

This form can be a bit confusing to fill out.  And if it is filled out incorrectly, cit ould mean failure to have your mail delivered or delivered on time!  Therefore, we hope to answer all of your questions here and demystify the form for you.

Who Can Fill out the USPS Form 1583?

The customer/business who is the ultimate mail recipient, is the applicant and the one that must fill out, and execute (sign) the form.   The applicant must execute this form in duplicate.  Also, the execution must be in the presence of the agent, his or her authorized employee, or a notary public.   In other words, there must be two original copies executed. An alternative option is to use a E-Notary service like the one we provide at Starthub.

The CMRA will provide the original completed and signed Form 1583 to the United States Postal Service. Your CMRA will also retain the duplicate completed signed copy at their business location where the mail is received. 

It’s important to note that your CMRA copy of the Form 1583 must be available at all times for examination by the postmaster.

Be sure to check with your CMRA for any additional requirements specific to that agent.

How do you complete Form 1583?

First, you will need two forms of identification to complete the form.  Acceptable forms of identification include:

  • valid driver’s license or state non-driver’s identification card; 
  • armed forces, government, university, or recognized corporate identification card; 
  • passport, alien registration card or certificate of naturalization; 
  • current lease, mortgage or Deed of Trust; 
  • voter or vehicle registration card; 
  • or a home or vehicle insurance policy

A photocopy of your identification may be retained by the CMRA for verification.

Next, fill out the form with name and address information.  In Box 2, the Mail recipient is either you, the individual or the Company.  You must use your full legal name or the full legal business name.  

Box 6 is the Applicant.  If you are the recipient (Box 2) AND the applicant, it will be the same.  However, if you filled out Box 2 as the Company, then Box 6 will now be different.  Box 6 must be the individual applicant’s name.

Boxes 9-14 are for that Business’ Company information.  If the Mail Recipient (Box 2) is a Business, then this is where the company information goes.

Finally, Box 15 is where you need to be with your CMRA representative to sign this or a Notary Public.  Check with your CMRA for how to execute this.  By signing this, the addressee and the agent agree to comply with all applicable Postal Service rules and regulations relative to delivery of mail through an agent. 

How do you submit Form 1583?

Your CMRA will likely submit the fully executed form to the USPS on your behalf and keep a copy for their records.  Be sure to consult with them to ensure that you are fully aware of the steps to submit.
